Abstract | The primary step of ¡§Chain of Survival¡¨ is¡¨activation of Emergency Response¡¨, which simply refers to the immediate report of the discovery of an OHCA (out-of-hospital-cardiac-arrest). Research in modern medical science has proven that performing CPR within the golden period of resuscitation shows a substantial increase in survival rate. However, it takes time for Emergency Medical Technicians (EMTs), to arrive at the location of the incident.As a result, someone arrounding at location should react during the golden period of resuscitation. Therefore, to increase the likelihood as well as the quality of bystanders to perform CPR, The National Fire Agency, Ministry of the Interior of Taiwan has been promoting the¡¨DA-CPR(dispatched assisted-CPR) Tightening the Chain of Survival¡¨program since 2017. The program has been revised and updated periodically with improvements such as performance measures to monitor effectiveness, hoping to provide OHCA patience that needs CPR treatment during the golden period. This paper aims to investigate whether a relationship between the use of DA-CPR and the survival rate of OHCA patience can be established. The research data are OHCA cases undertaken by Taipei City Fire Department between 2018 and 2019. The research can be divided into 2 parts. The first part examines whether or not an OHCA incident combined with the execution of DA-CPR shows a significantly higher survival rate than an OHCA incident with an absence of DA-CPR. The second part analyzes the relationship between the DA-CPR¡¦s quality of performance and the survival rate of OHCA patience, with a cross-national study comparing DA-CPR performance measure set by American Heart Association to Taiwan¡¦s performance measures. The results have shown the survival rates of OHCA patience increased as the use of DA-CPR. The result also suggests that the index of quality performance of DA-CPR has some deficiency which can not reflect the survival rates. However, the latter finding may be the result of the inability of the performance measures currently used by Taiwan to effectively measure quality and performance. Compared to Foreign performance measures, the index used in Taiwan still has a long way to go. To sum up, this research can be useful to identify the weakness of current DA-CPR as well as the limitations of current performance measures and used as a reference in improving the education, performance, quality, and performance measure of DA-CPR. |
